La rotura traumática, simultánea y bilateral del tendón cuadricipital es una lesión infrecuente, generalmente asociada a otras enfermedades sistémicas tales como insuficiencia renal o trastornos endocrinos. Presentamos el caso de un varón sano y atleta de 38 años que sufrió esta lesión mientras realizaba una sentadilla en el gimnasio. (AU)


The traumatic bilateral and simultaneous quadriceps tendon rupture is a rare injury, usually associated with other systemic diseases such as renal insufficiency or endocrine disorders. We present the case of a 38-year-old healthy male athlete who sustained this injury while performing a squat at the gym. (AU)

ABSTRACT

SUMMARY: Skeletal muscles play a fundamental role in people's lives and their evaluation provides significant information on health. Different tools have been used to evaluate muscle mass, and the evaluation of muscle thickness (MT) using ultrasound has been included as an alternative, which can be performed with the probe in different positions; however, these could present differences. The objectives of this study were to determine whether there are differences in the measurement of MT in the vastus lateralis (VL) muscle using the probe in the longitudinal or transverse position, and to determine its association with the lean mass of the lower limbs. The results indicated no significant differences between MT measurements with the probe in the longitudinal and transverse positions (p =0.084). However, when associating these measurements with lower limb lean mass, it was found that transverse measurements had a strong association (r =0.547; p < 0.001), while longitudinal measurements had a moderate association (r =0.351; p =0.007). This suggests that measurements with the probe positioned transversely to measure the MT would be the best option. Therefore, it could be useful as an indicator of lower limb lean mass in the absence of tools, such as bioelectrical bioimpedance or magnetic nuclear resonance.


El músculo esquelético cumple un rol fundamental en la vida de las personas, y su evaluación entrega mucha información de la salud. Se han utilizado diferentes herramientas para evaluar la masa muscular, y el último tiempo se ha incluido la evaluación del grosor muscular (MT) a través de la ecografía como una alternativa para estimarla, las cuales se pueden realizar con la sonda en distintas posiciones, sin embargo, estas podrían presentar diferencias. Los objetivos del estudio fueron determinar si existen diferencias en la medición de MT en el músculo vasto lateral (VL) utilizando la sonda en posición longitudinal o transversal y determinar su asociación con la masa magra de los miembros inferiores. Los resultados indican que no existen diferencias significativas entre las mediciones de MT con la sonda en posición longitudinal y transversal (valor p: 0.084). Sin embargo, al asociar estas mediciones con la masa magra de los miembros inferiores, se encontró que las mediciones transversales poseen una asociación fuerte (r: 0.547; valor p < 0.001), mientras que las mediciones longitudinales presentan una asociación moderada (r: 0.351; valor p: 0.007). Esto sugiere que las mediciones con la sonda posicionada transversal para medir MT serían la mejor opción. Por lo tanto, podría ser de utilidad como un indicador de masa magra de los miembros inferiores en caso de no contar con herramientas como la bioimpedancia bioeléctrica o resonancia nuclear magnética.

ABSTRACT

This paper summarizes professor GUAN Ling's clinical experience in the treatment of knee osteoarthritis (KOA) with structure-based medical acupuncture (SMA). Based on anatomy and biomechanics and through accurate physical examination, SMA adjusts the mechanical imbalance of muscles to relieve KOA dysfunction, and releases nerve compression to attenuate pain symptoms of KOA. In reference to traditional acupoint selection, and in association with painful areas and mechanical deduction, ashi points located at the rectus femoris, vastus intermedius, vastus medialis and vastus lateralis muscles, etc. are specially stimulated with acupuncture; and the rehabilitation training and health education are the adjuvant treatment for the patients.

Abstract Introduction Chronic obstructive pulmonary disease (COPD) is characterized by limited airflow associated with inflammatory response and systemic manifestations, such as dyspnea, as well as physical inactivity and intolerance to exercise. The sum of these changes can lead to peripheral muscle fatigue and exert an impact on the performance of activities of daily living (ADL). Objective To investigate the possible association between peripheral muscle fatigue and performance on ADL in individuals with COPD, and to compare the results to those of healthy age-matched individuals. Methods Individuals with a diagnosis of COPD and healthy volunteers aged 60 years or older were submitted to evaluations of peripheral muscle fatigue (using surface electromyography) and performance on the Glittre-ADL test. Results Nine individuals with COPD and ten controls were evaluated. Median isometric quadriceps contraction time was 72 [38] and 56 [51] seconds, respectively. Execution time on the ADL test was 6.1 [4] and 3.6 [1.3] minutes for COPD and control group respectively, with a significant difference between groups (p < 0.05). However, no significant correlation was found between the evaluations. Conclusion No association was found between quadriceps muscle fatigue and performance on ADL in the sample studied. In the intergroup comparison, the individuals with COPD exhibited worse ADL time execution, but no significant difference was found regarding quadriceps muscle fatigue.


Resumo Introdução A doença pulmonar obstrutiva crônica (DPOC) é caracterizada por fluxo aéreo limitado associado à resposta inflamatória e manifestações sistêmicas, como dispneia, além de inatividade física e intolerância ao exercício. A soma dessas alterações pode levar à fadiga muscular periférica e exercer impacto no desempenho das atividades de vida diária (AVD). Objetivo Investigar a possível associação entre fadiga muscular periférica e desempenho em AVD em indivíduos com DPOC e comparar os resultados com indivíduos saudáveis da mesma faixa etária. Métodos Indivíduos com diagnóstico de DPOC e voluntários saudáveis com idade igual ou superior a 60 anos foram submetidos a avaliações de fadiga muscular periférica (por meio de eletromiografia de superfície) e desempenho no teste Glittre-ADL. Resultados Foram avaliados nove indivíduos com DPOC e dez controles. O tempo médio de contração isométrica do quadríceps foi de 72 [38] e 56 [51] segundos, respectivamente. O tempo de execução do teste de AVD foi de 6,18 [4,09] e 3,67 [1,3] minutos para DPOC e grupo controle, respectivamente, com diferença significativa entre os grupos (p < 0,05). No entanto não encontrou-se correlação significativa entre as avaliações. Conclusão Não encontrou-se associação entre a fadiga muscular do quadríceps e o desempenho nas AVD na amostra estudada. Na comparação intergrupos, os indivíduos com DPOC apresentaram pior tempo de execução das AVD, mas não encontrou-se diferença significativa em relação à fadiga muscular do quadríceps.

OBJETIVO Determinar la resistencia máxima a la tracción (RMT) de la continuación prepatelar del cuádriceps (CPC).MATERIALES Y MÉTODOS Se realizó un estudio en cadáveres humanos. Fueron incluidos diez especímenes, en los cuales se utilizaron los tercios proximal y medial de la cortical anterior de la patela para evaluar las propiedades mecánicas de la CPC. En cada espécimen, se estudió un área de sección transversal de 0,2 cm2 (A1) y 1 cm2 (A2). Se aplicó una carga gradual para determinar la RMT.RESULTADOS La mediana de la RMT en el A1 fue de 232,56 N (rango: 141,23 N a 295,33 N) y en el A2 fue de 335,30 N (rango: 216,45 N a 371,40 N). El incremento en la TMR fue significativo entre las 2 áreas (p = 0,006).CONCLUSIÓN El ignificado clínico de este estudio es que la CPC es un tejido fuerte que puede servir de anclaje seguro para reconstrucciones alrededor de la patela. Un área relativamente pequeña tolera al menos 140 N y, a medida que crece el área, también aumenta la RMT.


OBJETIVE To determine the ultimate tensile strength (UTS) of the prepatellar quadriceps continuation (PQC). MATERIALS AND METHODS A human cadaveric study was performed. Ten fresh-frozen specimens were used. The proximal and medial thirds of the anterior cortex of the patella were used to assess the mechanical properties of the PQC. In each specimen, transverse section areas measuring 0.2 cm2 (A1) and 1 cm2 (A2) were studied. A gradual load was applied to determine the UTS. RESULTS The median UTS of A1 was of 232.56 N (range: 141.23 N to 295.33 N), and that of A2 was of 335.30 N (range: 216.45 N to 371.40 N). The increment in UTS was significant between the 2 areas (p » 0.006). CONCLUSION The clinical significance of the present study lies in the fact that it shows that the PQC is a strong tissue that can be a safe anchor for reconstruction around the patella. A relatively small area supports at least 140 N, and, as the area grows, the UTS increases as well.

ABSTRACT

Objetivos: Comunicar los resultados clínicos y funcionales de pacientes con síndrome pospolio sometidos a una artroplastia total de rodilla y evaluar si la elección de la prótesis está condicionada por el déficit funcional del cuádriceps. Materiales y Métodos: Se evaluó a pacientes con síndrome pospolio en su condición de déficit muscular y con escalas funcionales de tiempo de caminata en 10 m, distancia caminada en 2 min, y tiempo levántate y anda, y el Knee Society Score antes de la artroplastia de rodilla y un año después. Resultados: Los valores funcionales que impactaban directamente en la calidad de vida mejoraron notablemente en todos los pacientes. No se hallaron diferencias significativas en el Knee Society Score entre los casos más graves con déficit del cuádriceps con recurvatum y aquellos con menor afectación, pero las escalas funcionales sí mostraron una diferencia significativa entre estos subgrupos. Conclusiones: La artroplastia total de rodilla es una alternativa terapéutica válida en esta compleja enfermedad, que logra aliviar el dolor, recupera la función y mejora la calidad de vida. La restauración de la estabilidad mediante diseños de prótesis constreñidas es un factor clave en la recuperación de un patrón de marcha funcional en los pacientes con recurvatum. Los pacientes con fuerza del cuádriceps que vencen la resistencia de la gravedad tienen un resultado funcional equiparable al de los pacientes sin síndrome pospolio y no requieren prótesis abisagradas. Objectives: To report the clinical and functional outcomes of patients affected with post-polio syndrome (PPS) treated with total knee arthroplasty, evaluating whether the choice of the prosthesis is conditioned by the quadriceps functional deficit. Materials and Methods: Patients with PPS were evaluated in their muscle deficit condition and with functional tests such as 10-meter walk test (10MWT), 2-minute walk test (2MWT) and timed up and go test (TUG), as well as KSS preoperatively and 1 year after knee arthroplasty. Results: All patients significantly improved functional values that directly impacted their quality of life. The KSS did not present significant differences between the most severe cases with quadriceps deficit with recurvatum and those with less involvement, but the functional tests did show a significant difference between these subgroups. Conclusions: Total knee arthroplasty is a valid treatment alternative in this complex pathology, providing the patient with pain relief, recovery of function and improving their quality of life. The restoration of stability through constrained prosthetic designs is a key factor in the recovery of a functional gait pattern in patients with recurvatum. Patients with quadriceps strength who overcome the resistance of gravity have functional outcomes comparable to non-PPS patients and do not require hinged prostheses. Level of Evidence: IV

Resumo A artéria femoral profunda, devido às suas características anatômicas, se encontra protegida da maioria dos traumatismos vasculares. Relatamos um caso de pseudoaneurisma de ramo perfurante da artéria femoral profunda, associado à fístula arteriovenosa, secundário a rotura completa do músculo vasto medial em paciente jogador de futebol. A ressonância magnética demonstrou lesão muscular associada a pseudoaneurisma, e a angiotomografia confirmou a presença de pseudoaneurisma associado a fístula arteriovenosa de ramo da artéria femoral profunda. Foi realizado tratamento endovascular da fístula através da embolização com micromolas fibradas e drenagem cirúrgica do hematoma muscular. O paciente evoluiu bem, sem queixas clínicas no 30º dia de pós-operatório e também após 1 ano.


Abstract Due to its anatomical characteristics, the deep femoral artery is protected from most vascular injuries. We report a case of a soccer player with pseudoaneurysm of a perforating branch of the deep femoral artery, associated with an arteriovenous fistula and secondary to complete rupture of the vastus medialis muscle. Magnetic resonance imaging showed muscle damage associated with a pseudoaneurysm and angiotomography confirmed the presence of a pseudoaneurysm associated with a deep arteriovenous fistula of a branch of the deep femoral artery. Endovascular treatment of the fistula was performed by embolization with fibrous microcoils and surgical drainage of the muscle hematoma. The patient recovered well, was free from clinical complaints on the 30th postoperative day and also after 1 year.

Abstract Aim: The present study aimed to screen for differences in isokinetic peak torque, hamstrings-to-quadriceps ratio, and proprioception within the lower limbs of female handball athletes. Methods: Twelve college-level female handball athletes with no previous experience with resistance training performed five maximal isokinetic contractions of the knee extensors and knee flexors to determine isokinetic peak torque and hamstring-to-quadriceps ratios. Proprioception was determined by assessing passive position sense on an isokinetic dynamometer. Results: The athletes presented significantly greater (p < 0.01) knee extensors isometric peak torque for the jumping limb (144.9 ± 23.1) when compared to the non-jumping limb (132.9 ± 21.5). The Hamstrings-to-quadriceps ratio was below 0.6 for both limbs, being significantly greater (p < 0.01) for the non-jumping limb (0.56 ± 0.08) when compared to the jumping limb (0.50 ± 0.08). Conclusion: Female handball athletes that do not engage in resistance training can experience functional bilateral asymmetries in the knee extensors and knee-joint instability, as assessed by the hamstrings-to-quadriceps ratio due to the asymmetric characteristics of handball. Regular strength training might correct such asymmetries and instabilities.

OBJECTIVE@#To observe the effect of acupotomy on the morphology and ultrastructure of rectus femoris muscle in rabbits with knee osteoarthritis and to reveal the possible therapeutic mechanism involved in the effect of acupotomology on the treatment of knee osteoarthritis(KOA).@*METHODS@#Twenty-four male New Zealand rabbits aged 6 months and weighed (2.0±0.5) kg were randomly divided into blank group, model group and acupotomy group, 8 rabbits in each group. KOA model was established by modified Videman method with left hind limb extended plaster immobility for 6 weeks. In acupotomy group, the transfascial focal points of quadriceps femoris muscle were released by acupotomy under the guidance of Jingjin theory for 4 times and once a week, and the treatment points include Hedingci, Binwaixia, Binneixia. Blank group and model group were fed normally without intervention. One week after the end of the intervention, the pennation angle(PA), muscle thickness(MT), cross-sectional area(CSA) and strain ratio(SR) of rectus femoris were measured by ultrasound. HE staining was used to observe the changes of the tissue morphology, the number of muscle fibers and the average area of muscle fibers. The myofibril of rectus femoris, sarcomere and myofilament were observed by transmission electron microscope.@*RESULTS@#The PA of rectus femoris muscle in the blank group was (9.05±0.21)°. The MT was(1.09±0.09) cm and the CSA was(1.30±0.01) cm2. The PA of rectus femoris muscle in the model group was (3.06±0.15)°. The MT was (0.71±0.02) cm and the CSA was(0.77±0.02) cm2. The PA of rectus femoris muscle in the acupotomy group was (6.94±0.28)°. The MT was (0.80±0.05) cm and the CSA was(0.94±0.03) cm2. The muscle PA, MT and CSA of rectus femoris in the model group were significantly smaller than those in the blank group (P<0.05). Those in acupotomy group were significantly increased compared with those in model group (P<0.05). The SR of rectus femoris muscle was 1.19±0.02 in the blank group, 3.50±0.05 in the model group and 1.99±0.07 in the acupotomy group. The elastic SR of the model group was significantly higher than that of the blank group (P<0.05). These in acupotomy group was significantly lower than that in model group(P<0.05). The results of HE staining showed:in blank group, the fascicles of rectus femoris were arranged neatly, the number of beam of muscle fibers within the fixed visual field was 94.38±3.50 and the average CSA was(0.75±0.22) mm2. In model group, the fascicles of rectus femoris with different sizes were disorganized with a small amount of inflammatory cell infiltration, the number of beam of muscle fibers within the fixed visual field was 196.63±2.62 and the average CSA was(0.26±0.03) mm2. Compared to the blank group, a significant increase in the number of muscle fibers in the fixed field in the model group (P<0.05) and the average CSA decreased significantly(P<0.05). In acupotomy group, the rectus femoris fascicles in the acupotomy group tended to be arranged in a more orderly manner, with the inflammatory cells decreased, the number of beam of muscle fibers within the fixed visual field was 132.88±4.61 and the average CSA was(0.70±0.07) mm2. Compared to the model group, a significant decrease in the number of muscle fibers in the fixed field in the model group(P<0.05) and the average CSA increased significantly(P<0.05). The results of transmission electron microscope showed:compared with the blank group, the overall arrangement of the myofibrils of the rectus femoris in the model group was less structured. There was fracture between the muscle fibers and the sarcomere, the myofilaments were disordered, and the fracture of the Z line was discontinuous. Compared with the model group, the myofibrillar texture of rectus femoris in acupotomy group was clearer, and the Z line was more continuous.@*CONCLUSION@#Based on the jingjin theory, the release of quadriceps femoris by acupotomy can effectively improve the morphology and structure of rectus femoris, and promote the repair and reconstruction of chronic skeletal muscle injury in rabbits with KOA, which may be one of the mechanisms of acupotomy in the treatment of KOA.

ABSTRACT

Objective: To explore the effects of bicycle ergometer rehabilitation training on quadriceps and walking ability of patients with lower limb dysfunction caused by extensive burns. Methods: A prospective randomized controlled study was conducted. A total of 40 patients with extensive burns who met the inclusion criteria and were admitted to Tongren Hospital of Wuhan University&Wuhan Third Hospital from December 2017 to December 2020 were selected. According to the random number table, the patients were divided into conventional training group (16 males, 4 females, aged (45±10) years) and combined training group (13 males, 7 females, aged (39±8) years). Patients in conventional training group were given conventional rehabilitation therapy such as joint loosening, lower limb strength training, walking training, and pressure therapy, while patients in combined training group were given additional bicycle ergometer rehabilitation training on the basis of conventional rehabilitation. For patients in the 2 groups before and after a 2-month's treatment, the thickness of quadriceps was measured by ultrasonic diagnostic instrument, the muscle strength of quadriceps was measured by portable muscle strength tester, the walking ability was tested with a 6-min and a 10-meter walk tests, and the patients' satisfaction for treatment effects was assessed using the modified Likert scale. Data were statistically analyzed with independent or paired sample t test, Mann-Whitney U test, Wilcoxon signed rank test, or chi-square test. Results: After 2-month's treatment, the quadriceps thickness of patients in combined training group was (3.76±0.39) cm, which was significantly thicker than (3.45±0.35) cm in conventional training group (t=2.67, P<0.05); quadriceps thickness of patients in conventional training group and combined training group after 2-month's treatment was significantly thicker than that before treatment (with t values of 5.99 and 8.62, respectively, P<0.01). After 2-month's treatment, the quadriceps muscle strength of patients in combined training group was significantly greater than that in conventional training group (Z=2.69, P<0.01); quadriceps muscle strength of patients in conventional training group and combined training group after 2-month's treatment was significantly greater than that before treatment (with Z values of 3.92 and 3.92, respectively, P<0.01). After 2-month's treatment, the 6-min walking distance of patients in combined training group was (488±39) m, which was significantly longer than (429±25) m in conventional training group (t=5.66, P<0.01); the 6-min walking distance of patients after 2-month's treatment in conventional training group and combined training group was significantly longer than that before treatment (with t values of 13.16 and 17.92, respectively, P<0.01). After 2-month's treatment, the 10-meter walking time of patients in combined training group was significantly shorter than that in conventional training group (t=3.20, P<0.01); and the 10-meter walking time in conventional training group and combined training group was significantly shorter than that before treatment (with t values of 7.21 and 13.13, respectively, P<0.01). The patients' satisfaction score for treatment effects in combined training group was significantly higher than that in conventional training group (Z=3.14, P<0.01), and the patients' satisfaction scores for treatment effects in conventional training group and combined training group after 2-month's treatment were significantly greater than those before treatment (with Z values of 3.98 and 4.04, respectively, P<0.01). Conclusions: Bicycle ergometer rehabilitation training can be used to improve quadriceps thickness, muscle strength, and walking ability of patients with lower limb dysfunction caused by extensive burns. It can also improve the satisfaction of patients with the treatment outcome, and therefore is worthy of promotion.

La posición de la articulación puede generar cambios en el reclutamiento muscular. De esa forma el presente estudio tiene como objetivo verificar si variaciones en las posiciones del pie durante la extensión de las rodillas son eficaces para la selección específica de los músculos recto femoral, vasto lateral y vasto medial, que componen el CF. Once mujeres universitarias (edad 22.36 ± 3.56 años) participaron del estudio. Fue evaluada la activación de los músculos a través de la electromiografía de superficie, con una configuración bipolar, en 3 series de 10 repeticiones máximas (RM) de extensión bilateral de rodilla, en tres posiciones de los pies: neutra, rotación interna y rotación externa. Los resultados no presentaron diferencias significativas entre los músculos del CF en todas las posiciones del pie. En conclusión, los resultados del presente estudio sugieren que las variaciones en los grados de rotación de los pies durante la extensión de las rodillas no son eficaces para la selección específica de los músculos que componen el CF.

O fortalecimento do grupo muscular quadríceps femoral (QF) é muito importante devido sua função para o movimento. Mudanças na posição do pé, podem gerar alterações no recrutamento muscular, desta forma o presente estudo objetivou verificar se as variações nas posições dos pés, durante a extensão de joelhos, são eficazes para a seleção específica dos músculos reto femoral, vasto lateral e vasto medial, que compõem o QF. Onze mulheres universitárias (idade 22.36 ± 3.56 anos) participaram do estudo. Foi avaliada a ativação dos músculos através da eletromiografia de superfície, com configuração bipolar, em 3 séries de 10 repetições máximas (RM) de extensão de joelhos bilateral, em três posições dos pés: posição neutra, rotação interna e rotação externa. Os resultados não apontam diferenças significativas entre os músculos do QF superficial em todas as posições dos pés. Em conclusão, os resultados do presente estudo sugerem que as variações nos graus de rotação dos pés durante a extensão de joelhos, não são eficazes para a seleção específica dos músculos que compõem o QF.

The joint position may generate modifications in muscle recruitment. Consequently, the objective of this study was to verify if foot position variations during knee extension are efficient in the specific selection of the rectus femoris, vastus lateralis, and vastus medialis muscles comprising the FQ. Eleven female college students (ages 22.36 ± 3.56 years old) participated in the study. Muscle activation was evaluated through surface electromyography, with bipolar configuration, in 3 series of 10 maximum repetition (MR) of bilateral knee extension, in three foot positions: neutral position, internal rotation, and external rotation. Results do not indicate significant differences among the superficial FQ muscles in all foot positions. In conclusion, results suggest that variations in foot rotation degrees during knee extension are not efficient for the specific selection of the muscles comprising FQ.

Introducción: El aumento de la población adulta mayor es uno de los grandes cambios demográficos en Chile, por lo que resulta necesario establecer abordajes terapéuticos eficaces para prevenir caídas y, consecuentemente, la dependencia funcional en este grupo etario. Objetivo: Evaluar la eficacia de un programa kinésico de prevención de caídas mediante fortalecimiento muscular de extremidad inferior en adultos mayores de 65 años con alteración del equilibrio, en comparación con el programa kinésico convencional. Métodos: Ensayo clínico aleatorizado no farmacológico, incluyó un grupo experimental y un grupo control con 25 participantes en cada grupo. La modalidad del tratamiento experimental es más educativa y participativa que el convencional. La eficacia de cada tratamiento fue evaluada mediante la variación de parámetros de equilibrio estático y dinámico al segundo y tercer mes de tratamiento (resultados principales). Resultados: Los resultados secundarios fueron: fuerza, potencia y diámetro pierna - muslo. Se evaluó la eficacia de ambos programas por el modelo multinivel, se consideraron las variables tiempo y tratamiento. Resultados: Hubo mejorías significativas en parámetros de equilibrio estático y dinámico con ambos tratamientos. La eficacia del tratamiento experimental fue superior a la del convencional en parámetros de potencia muscular y aumento de diámetros de ambos muslos y pierna izquierda. El factor tiempo de tratamiento aportó significativamente a la eficacia. Conclusiones: Ambos tratamientos mejoran los parámetros de equilibrio, pero el tratamiento experimental es más eficaz sobre parámetros de potencia muscular. Con su modalidad educativa y participativa este tratamiento podría optimizar resultados para prevenir caídas del adulto mayor en el nivel primario de atención(AU)


Introduction: The increase in the older adult population is one of the great demographic changes in Chile, a reason why it is necessary to establish effective therapeutic approaches to prevent falls and, consequently, functional dependence in this age group. Objective: To assess the efficacy of a kinesthetic program for the prevention of falls through lower limb muscle strengthening in adults over 65 years of age with impaired balance, in comparison with the conventional kinesthetic program. Methods: Nonpharmacological randomized clinical trial including an experimental group and a control group, with 25 participants each. The modality of experimental treatment is more educational and participatory than the conventional one. The efficacy of each treatment was assessed based on the variation of static and dynamic balance parameters at the second and third months of treatment (main results). Secondary outcomes included strength, power, and leg-thigh diameter. The efficacy of both programs was assessed using the multilevel model; the variables time and treatment were considered. Results: There were significant improvements in static and dynamic balance parameters with both treatments. The efficacy of the experimental treatment was higher than the conventional one in parameters of muscular power and increase in diameters of both thighs and the left leg. The factor time of treatment contributed significantly to efficacy. Conclusions: Both treatments improve balance parameters, but the experimental treatment is more effective regarding muscle power parameters. With its educational and participatory modality, this treatment could optimize results to prevent falls in the older adult at the primary level of care(AU).

ABSTRACT Introduction: Near infrared spectroscopy (NIRS) is a non-invasive technique that is used in the assessment of tissue oxygenation and the monitoring of physical activity. Objective: To determine the influence of sexual, anthropometric and ergospirometric factors on muscle oxygenation of the quadriceps and gastrocnemius, obtained by NIRS during a stress test. Methods: Twenty healthy subjects participated in this study (10 women). Two Humon Hex® devices were placed on the dominant side of the quadriceps and gastrocnemius muscles to measure muscle oxygen saturation (SmO2). The stress test was performed on a treadmill with electrocardiographic control and measurement of oxygen consumption. SmO2 was obtained at rest and after maximum effort during the stress test. In addition, the height, weight, skinfold and waist contour were measured. Bioimpedance was used to obtain the percentages of fat mass and muscle mass, which were used to calculate the relative fat mass (RFM). Results: The SmO2 of both muscles at rest is higher in males than in females. At maximum effort, the SmO2 of the quadriceps is similar in both groups. The SmO2 of both muscles is positively related to height, body mass, percentage of mass muscle and waist contour, and negatively with percentage of mass fat, RFM and skinfold thickness. The negative correlation between fat percentage and oxygen saturation is more evident in females. It was observed that the variables that quantify maximum effort are not related to the SmO2 values, except for the correlation between HR max and SmO2 of the gastrocnemius muscle in males. Conclusion: The SmO2 of recreational athletes is influenced by the location of the device and the fat mass of the subjects. The biggest differences between the sexes are in the gastrocnemius muscle. RESUMEN Introducción: La espectroscopia de infrarrojo cercano (NIRS) es una técnica no invasiva usada en la evaluación de la oxigenación tisular y en la monitorización de la actividad física. Objetivos: Determinar la influencia de factores sexuales, antropométricos y ergoespirométricos sobre la oxigenación muscular de cuádriceps y del gastrocnemio, obtenidos por NIRS durante un test de esfuerzo. Métodos: Participaron en este estudio 20 individuos saludables (10 mujeres). Se colocaron dos dispositivos Humon Hex® en el lado dominante de los músculos cuádriceps y gastrocnemio para medir la saturación de oxígeno muscular (SmO2). El test de esfuerzo fue realizado en una cinta ergométrica con control electrocardiográfico y medición del consumo de oxígeno. La SmO2 fue obtenida en reposo y después de esfuerzo máximo durante el test. Además, fueron medidos altura, peso, pliegues cutáneos y contorno de la cintura. Fue usada bioimpedancia para obtener los porcentuales de masa grasa y masa muscular, que fueron usados para calcular la masa grasa relativa (MGR). Resultados: La SmO2 de ambos músculos en reposo es mayor en hombres que en mujeres. En el esfuerzo máximo la SmO2 del cuádriceps es semejante en los dos grupos. La SmO2 de ambos músculos es positivamente relacionada con altura, masa corporal, porcentual de masa muscular y contorno de la cintura y negativamente relacionada con el porcentaje de masa grasa, MGR y espesor de los pliegues cutáneos. La correlación negativa entre el porcentual de grasa y la saturación de oxígeno es más evidente en las mujeres. Se observó que variables que cuantifican el esfuerzo máximo no está relacionadas con los valores SmO2, excepto por la correlación entre FCmáx y SmO2 del músculo gastrocnemio en sexo masculino. Introdução: o músculo reto femoral é o músculo mais frequentemente lesado do grupo quadríceps durante chutes e corridas repetitivas, apesar de suas avulsões serem raras. A dor localizada na coxa proximal e a incapacidade de flexionar o quadril e/ou estender o joelho associado à história do paciente de contração violenta ou alongamento forçado do grupo muscular do quadríceps femoral podem indicar uma avulsão do reto femoral. Objetivo: relatar um caso e revisar a literatura acerca desta lesão incomum. Materiais e Métodos: revisão do prontuário, registro fotográfico do método diagnóstico e revisão da literatura. Resultados: homem de 46 anos com dor localizada no quadril esquerdo com irradiação para a coxa por três meses após cair no chão com a região dolorida em um jogo de futebol. A dor começou subitamente uma semana após a queda. A ressonância magnética do quadril apresentava avulsão completa do reto femoral esquerdo, distando 2 cm da espinha ilíaca anterior inferior com efusão líquida adjacente. Conclusão: este relato demonstra a dificuldade de diagnosticar essa lesão, pois o seu exame físico é inespecífico, podendo simular patologias mais complexas, necessitando de exames complementares para seu correto diagnóstico.


Introduction: the rectus femoris muscle is the most frequently injured muscle of the quadriceps group during repetitive kicking and sprinting. Avulsions of the rectus femoris are rare injuries. Pain located at the proximal thigh and disability in flexing the hip and/or extending the knee associated with a patient history of a violent contraction or forceful stretching of the quadriceps femoris muscle group can indicate an avulsion of rectus femoris. Objective: report a case and review the literature about this uncommon lesion. Materials and Methods: we reviewed medical records, photographic records of diagnostic methods, and reviews from the literature. Results: a 46-year-old man with pain located at the left anterior hip with irradiation to the thigh for three months after falling to the ground with the sore region in a soccer game. The pain started suddenly one week after the fall. The MRI of the hip featured complete avulsion of the left rectus femoris, being 2 cm distal from the anterior inferior iliac spine with adjacent liquid effusion. Conclusion: this report demonstrates the difficulty of diagnosing this lesion, since its physical examination is non-specific, and it can simulate more complex pathologies, requiring complementary tests for its correct diagnosis.

SUMMARY: Anatomical studies describe the vastus medialis (VM) as being subdivided into two morphologically distinct components, the vastus medialis obliquus (VMO) and the vastus medialis longus (VML). However, there are discrepancies regarding the functional differentiation of these components. The aim of this study was to compare the levels of activation of the VMO and the VML by high density surface electromyography. Twelve healthy young women (age: 21.4 ± 2.0 years; weight: 58.1 ± 7.5 kg; height: 1.6 ± 0.1 m), performed an open kinetic chain knee exercise during which the EMG activity of the VMO and the VML was recorded with two- dimensional matrices of 32 surface electrodes. The exercises were performed with three levels of resistance (5, 10 and 15 % of the body weight (BW)), considering three phases: concentric, isometric and excentric. In the isometric phase the VMO had greater activation than the VML with the three levels of resistance (p<0.05). In the excentric phase, the VMO also showed greater activation than the VML with the 10 and 15 % BW resistance levels, while in the concentric phase, the VMO showed greater activity than the VML with only the 15 % BW resistance. The results indicated significant differences in the activation level of the two components of the VM. This bears importance in the development of exercises intended to achieve a greater or more selective activation of the VMO. In the sample subjected to evaluation, the EMG recordings describe a greater activation of the VMO in comparison to the VML, which is more important in the isometric and excentric phases of the flexion/extension of the knee in an open kinetic chain. These findings suggest a functional compartmentalization of the VM.


RESUMEN: Los estudios anatómicos describen que el músculo vasto medial (VM) se subdivide en dos componentes morfológicamente distintos, el vasto medial obliquus (VMO) y el vasto medial largo (VML). Sin embargo, existen discrepancias con respecto a la diferenciación funcional de estos componentes. El objetivo de este estudio fue comparar los niveles de activación del VMO y el VML mediante electromiografía de superficie de alta densidad. Doce mujeres jóvenes sanas (edad: 21,4 ± 2,0 años; peso: 58,1 ± 7,5 kg; altura: 1,6 ± 0,1 m), realizaron un ejercicio de rodilla de cadena cinética abierta durante el cual se registró la actividad EMG de la VMO y la VML con dos matrices dimensionales de 32 electrodos de superficie. Los ejercicios se realizaron con tres niveles de resistencia (5, 10 y 15% del peso corporal (PC)), considerando tres fases: concéntrica, isométrica y excéntrica. En la fase isométrica el VMO tuvo mayor activación que el VML con los tres niveles de resistencia (p <0,05). En la fase excéntrica, el VMO también mostró mayor activación que el VML con los niveles de resistencia de 10 y 15% BW, mientras que en la fase concéntrica, el VMO mostró mayor actividad que el VML con solo el 15 % de resistencia al BW. Los resultados indicaron diferencias significativas en el nivel de activación de los dos componentes de la VM. Esto tiene importancia en el desarrollo de ejercicios destinados a lograr una activación mayor o más selectiva del VMO. En la muestra sometida a evaluación, los registros EMG describen una mayor activación del VMO en comparación con el VML, que es más importante en las fases isométrica y excéntrica de la flexión / extensión de la rodilla en cadena cinética abierta. Estos hallazgos sugieren una compartimentación funcional de la VM.

OBJECTIVE@#To investigate the correlation between the quadriceps cross-sectional area (CSA) and quadriceps muscle volume (QMV) at different horizontal levels from the upper edge of the patella, and to determine the best observation position.@*METHODS@#Thigh magnetic resonance imaging (MRI) images of 22 Chinese young men [age: (29±6) years] with anterior cruciate ligament (ACL) rupture were examined. The CSA was measured at 18, 15, and 12 cm above the upper edge of the pate-lla (denoted by CSA-18, CSA-15 and CSA-12 respectively), and the QMV and CSA were determined by semiautomatic segmentation. A curve model was established to estimate QMV. Bland-Altman analysis was performed to determine the confidence limits of the volumes.@*RESULTS@#On the unaffected side, the mean QMV was (1 944.45±323.77) cm3. The quadriceps CSA at the upper edge of the patella at 18, 15, and 12 cm was (80.80±12.16) cm2, (77.53±12.03) cm2, and (72.68±10.51) cm2, respectively. The coefficients of determination (R2), ascertained using curve estimation models, for the 3 positions were 0.819, 0.755, and 0.684 (P < 0.001), and the standard deviations of the volume estimated value (SEE) were 7.4%, 8.7%, and 9.8%. The fitting equations of the three horizontal positions were all good, but the fitting degree of CSA-18 was the highest. The Bland-Altman scatter plot showed that the arithmetic means of the QMV at 18, 15 and 12 cm from the upper edge of the patella 0.8 cm3, -1.1 cm3, and 0.9 cm3 and 95% limits of agreement (LoA) were (-268.8, 270.5), (-315.2, 313.1), and (-355.7, 357.5), respectively. The estimated QMV was in good agreement with the measured value. The difference between the estimated CSA-18 and measured values was the smallest. The results on the affected side were consistent.@*CONCLUSION@#The correlation between QMV and CSA in the young men with the upper edge of patella as baseline was reliable and consistent. Among them, CSA-18 had the highest correlation with the QMV. However, different observation sites could be selected for different injuries of the quadriceps.

OBJECTIVE@#To investigate the clinical results of the application of critical rehabilitation pathway in the rehabilitation after total knee arthroplasty.@*METHODS@#From March 2015 to December 2019, 67 patients with total knee arthroplasty (TKA) were included. There were 49 females and 18 males, 42 cases on the left and 25 cases on the right, with an average age of 60 to 81(70.72±5.92) years old. Critical rehabilitation paths included intensive strength and gait rehabilitation exercises. All patients were evaluated before operation and 3, 12 months after operation. The evaluation indexes included stair climbing test (SCT), six minute walk test (6MWT), quadriceps and hamstring strength, range of motion, visual pain scale (VAS), Western Ontario McMasterUniversity Osteoarthritis score(WOMAC).@*RESULTS@#All the patients completed the entire pathway and the assessment. The results of pre-operative, 3 months after surgery and 12 months after surgery were as follows respectively. SCT-up: (16.32±3.58) s, (18.16±2.46) s, (11.00±1.29) s, @*CONCLUSION@#Critical rehabilitation path is safe and effective. The knee function of patients who receive critical rehabilitation path after TKA is significantly improved in the first 12 months after operation.

OBJECTIVE@#To study mechanism of improvement of stress concentration on patellofemoral joint by stiletto needle releasing lateral patellar retinaculum guided by the theory of Jinshugu() and based on the finite element model of knee joint. and to elucidate the biomechanical mechanism of stiletto needle releasing changing patellar trajectory and reducing patellofemoral joint pressure.@*METHODS@#CT data of knee joint from a normal male (aged 29, heighted 171 cm, weighted 58 kg) was selected. Starting with construction of three-dimensional model of knee joint by using finite element software, the finite element model of knee joint with complete tendonand bone structures were established through several steps, such as geometric reconstruction, reverse engineering, meshing, material assignment and loading analysis. The loading condition was set as 500 N load on knee joint, and the average tensile stress of quadriceps femoris tendon was about 200 N. To simulate the release of lateral patellar retinaculum by stiletto needle at 30 and 90 position of knee flexion in finite element model separately, and to compare the improvement of stress concentration of patellofemoral joint by stiletto needle intervention under different knee flexion conditions.@*RESULTS@#The peak stress of patellofemoral joint and tibiofemoral joint decreased after stiletto needle releasing of patellofemoral lateral retinaculum compared with before intervention, which was(1) knee flexion at 30 degrees:patellar cartilage decreased by 0.498 MPa (decreased 9.06%), femoral trochlea decreased by 0.886 MPa(decreased 16.27%);(2) knee flexion at 90 degrees:patellar cartilage decreased by 0.558 MPa (decreased 8.6%), femoral trochlea decreasedby 0.607 MPa (decreased 9.94%).@*CONCLUSION@#Releasing lateral patellofemoral retinaculum with stiletto needle could effectively alleviate the stress concentration of patellofemoral joint and reduce local stress peak value, which it is helpful to improve patellar trajectory and make stress distribution more uniform.

ABSTRACT

Resumo O fortalecimento do grupo muscular quadríceps femoral (QF) é muito importante devido sua função para o movimento. Mudanças na posição do pé, podem gerar alterações no recrutamento muscular, desta forma o presente estudo objetivou verificar se as variações nas posições dos pés, durante a extensão de joelhos, são eficazes para a seleção específica dos músculos reto femoral, vasto lateral e vasto medial, que compõem o QF. Onze mulheres universitárias (idade 22.36 ± 3.56 anos) participaram do estudo. Foi avaliada a ativação dos músculos através da eletromiografia de superfície, com configuração bipolar, em 3 séries de 10 repetições máximas (RM) de extensão de joelhos bilateral, em três posições dos pés: posição neutra, rotação interna e rotação externa. Os resultados não apontam diferenças significativas entre os músculos do QF superficial em todas as posições dos pés. Em conclusão, os resultados do presente estudo sugerem que as variações nos graus de rotação dos pés durante a extensão de joelhos, não são eficazes para a seleção específica dos músculos que compõem o QF.


Abstract Strengthening the femoral quadriceps (FQ) muscular group is very important due to its function in moving. The joint position may generate modifications in muscle recruitment. Consequently, the objective of this study was to verify if foot position variations during knee extension are efficient in the specific selection of the rectus femoris, vastus lateralis, and vastus medialis muscles comprising the FQ. Eleven female college students (ages 22.36 ± 3.56 years old) participated in the study. Muscle activation was evaluated through surface electromyography, with bipolar configuration, in 3 series of 10 maximum repetition (MR) of bilateral knee extension, in three foot positions: neutral position, internal rotation, and external rotation. Results do not indicate significant differences among the superficial FQ muscles in all foot positions. In conclusion, results suggest that variations in foot rotation degrees during knee extension are not efficient for the specific selection of the muscles comprising FQ.


Resumen El fortalecimiento de los músculos del cuádriceps femoral (CF) es importante debido a su función en el movimiento. La posición de la articulación puede generar cambios en el reclutamiento muscular. De esa forma el presente estudio tiene como objetivo verificar si variaciones en las posiciones del pie durante la extensión de las rodillas son eficaces para la selección específica de los músculos recto femoral, vasto lateral y vasto medial, que componen el CF. Once mujeres universitarias (edad 22.36 ± 3.56 años) participaron del estudio. Fue evaluada la activación de los músculos a través de la electromiografía de superficie, con una configuración bipolar, en 3 series de 10 repeticiones máximas (RM) de extensión bilateral de rodilla, en tres posiciones de los pies: neutra, rotación interna y rotación externa. Los resultados no presentaron diferencias significativas entre los músculos del CF en todas las posiciones del pie. En conclusión, los resultados del presente estudio sugieren que las variaciones en los grados de rotación de los pies durante la extensión de las rodillas no son eficaces para la selección específica de los músculos que componen el CF.
